HeyForm

HeyForm is an open-source form builder for creating surveys, quizzes, polls, and other conversational forms. It collects information and feedback through interactive online forms and runs as a hosted service or self-hosted on your own infrastructure.

Forms support a wide range of inputs, from text, email, and phone number to picture choices, date pickers, and file uploads. Conditional logic and URL redirections make forms adapt to answers, and results export to CSV. Themes cover fonts, colors, backgrounds, and custom CSS, with detailed analytics on drop-off and completion rates.

Integrations connect to webhooks, analytics and marketing tools, Zapier, and Make.com. The official hosted service at my.heyform.net handles backups, security, and maintenance, while self-hosting supports one-click deploys on Railway, Zeabur, Sealos, and other platforms. HeyForm is licensed under AGPL-3.0.
