Fasten OnPrem

Fasten OnPrem is an open-source, self-hosted personal health record app for viewing and managing patient medical data. It is designed for individuals and families, not clinics, so medical history for household members can live in one private place you run yourself.

Records can be entered manually or uploaded as FHIR Bundles exported elsewhere; it does not import directly from healthcare providers or integrate with EHRs. Fasten supports the medical industry's FHIR protocol, includes condition-specific dashboards and diagnostic test tracking, and can be discovered by companion mobile apps and local network devices for syncing.

Fasten OnPrem runs with Docker Compose and starts with a local account. Multi-user household features are a work in progress, and the service owner has root access to all user records. It is community maintained and separate from Fasten Connect, so hosted infrastructure, support services, and commercial features are not included.
